CCG: Making Intermediate Care Better in Barking and Dagenham

Read here the latest CCG (Clinical Commissioning Groups) document that talks about intermediate care in Barking and Dagenham, Havering and Redbridge. It explains what the CCG have been doing during the past year to try out new ways of working and what they would like to do in the future to make those services better.
