London Ambulance Service and Mental Health and Dementia Service Users

The London Ambulance Service are keen to consider the views of patients and service users when developing their future plans. This year they are focusing in particular on mental health service users and patients with dementia. They would be very grateful if you could take some time to fill in the survey below if you access or know someone who is a mental health or dementia service user. You/They can choose to complete it either electronically and email it back to them at ppi@lond-amb.nhs.uk, or print it out and post it to them (no stamp needed) at the Freepost address provided on the document.
