Can you help NHS England London, to shape better community and social care services across the capital?

NHS England are asking service user groups, volunteer groups and local authority leaders to take part in an online conversation. They are trying to improve the way health and care services in the community work. These are the health services that are based outside of hospital settings, in clinics and who provide care in peoples’ homes.
If you have experience of these services, they would really like to hear what you think. People are able to sign up and join the conversation at the following link: https://transformldn.clevertogether.com
The online conversation opens on the 21st January 2015 and closes on the 12th February 2015.
